Health Information Management (HIM) department is here to help our patients and all patient care facilities/providers needing to obtain copies of medical records for all facilities owned and operated by Ascentist Healthcare.What is Health Information Management?
The Health Information Management department is your resource to getting your medical records, HIM is also responsible for organizing all your health care documents and records to make them available for your physicians to review. They also make your records available to any other physicians, your insurance company, or your attorney.Requesting Your Medical Records
To request a copy of your medical records please call Ascentist Healthcare Health Information (Medical Records) department at 913-529-1807 between 7:30 a.m. to 4 p.m., Monday through Friday.An “Authorization to Use and Disclose Protected Health Information” is the form you need to request a copy of your medical records. This form tells us to whom we are authorized to release your medical information (e.g. yourself or spouse, a physician or clinic, or another hospital) and what specific records are to be released.
Who may sign “Authorization to Use and Disclose Protected Health Information”?
The individuals who are allowed to sign an Authorization to Use and Disclose Protected Health Information form for medical records are:
- Individuals 18 or older. If the patient is a minor (less than 18), the form must be signed by a parent or a legal guardian* of the patient.
- Executor of an Estate. If the patient is deceased, the form must be signed by the executor of the patients estate. *
- Legal Representative. If the patient is unable to sign because of physical or mental incapacity, the form must be signed by the patients legal representative. *
* If you are signing on behalf of a patient for whom you are the legal guardian or personal representative, you must produce a certified copy of your appointment as legal guardian or personal representative. If you are signing on behalf of a patient who is deceased, you must provide a certified copy of the patient’s death certificate.
